Reporting to Motheread/Fatheread® Arizona
Your ongoing obligation to the Arizona Humanities Council, the Arizona affiliate of Motheread®, Inc., is to let us know what is happening at your site regarding schedules, demographics, and evaluation, so we can include this information in our calendars, grant proposals, and reports to funders and the Motheread® National Office. With or without AHC funding, you are still required to provide Motheread® Arizona with this information.
These mandatory reports are critical to tracking the impact and success of Motheread® Arizona programs, and will provide you, AHC, and the National Office with vital information about audience demographics and outcomes, which helps in attracting and remaining accountable to funders.
Schedule Report
Let us know ahead of time about any Motheread® adult series, so we may add them to our calendars and program statistics. This form is due before the start of each adult series.
Series Report
Submit this immediately after each adult series, so we know the audience we are reaching and what your programs looked like. This form is due within 30 days of the end of each adult series.
Match Report
Submit this immediately after each adult series. AHC is required to report matching figures to the National Endowment for the Humanities, so we need to know the value of all goods and services your organization has supplied to your Motheread® program, including staff time (including the Training Institute), facilities, materials, etc. Also include goods and services any outside organization has contributed to your Motheread® program.
(It may also help your public relations and grant proposals to include AHC's Motheread® contribution. Ask the Motheread® Arizona coordinator for this information and total amount.)
